FORMS FOR NEW JUNIORS PLAYERS

On your first visit to the club please bring:
  1. MWJFL Registration form
  2. MWJFL Media Release form
  3. Proof of age (see below for more information)

Please place them in an envelope marked 'Player Registration - attention Club Secretary'.
Note payment of fees, or proof of payment of fees (e.g. receipt from online payment), is required to complete registration

Transfer/clearance

A MWJFL Transfer/Clearance form is needed if the player has been registered at another MWJFL club or has been registered in another league (e.g. the MSJFL).  It is not needed if the player's past football experience has been Auskick, or school football.

Proof of age

On first registering with the MWJFL all players must provide 'proof of age'.
Families are asked to provide a photocopy or email us a scanned copy of one of the following documents:
Birth Certificate/ Passport/ Baptism Certificate/ School Card/ CAFHS or CYH Blue Book/ School letter (from Principal, on school letterhead)/ Ambulance Fund subscription or the like.
The proof of age document must include the player’s full name and date of birth.

PLEASE NOTE: Do not offer the original documents to us.  The photocopy/scanned image is sent away with the registration documents to the MWJFL
